GLP-1 weight program

Proteomic Response Report · Clinician edition

How this patient's biology responded to 12 weeks of semaglutide.

About 1,000 plasma proteins measured at four timepoints on one plate, read against the patient's own baseline noise, and anchored to accredited clinical chemistry.

Three things to know before the next visit.

Improved

Metabolic, inflammatory, hepatic and endothelial markers moved together and agree with the clinic HbA1c, ALT, lipids and hs-CRP.

Unchanged

No kidney or neuro marker left the patient's own noise band. Reported as a result, not an omission.

Review with the patient

A muscle/bone pattern consistent with lean-mass loss, and a gut-epithelial marker that rose during the nausea weeks. Neither is diagnostic.

Each bar is the mean shift of an organ system’s proteins, in multiples of this patient’s own within-person noise, signed so that right = favourable. Bars inside the shaded zone are indistinguishable from no change. This is not a biological age and is not comparable across patients.

Week 16–18, after 4+ weeks at a stable dose. Re-check muscle/bone and gut panels; consider DXA before the next dose step.

Reading rule. Teal = moved in the direction the program intends. Stone = did not move beyond this patient’s own noise. Amber = moved in a direction worth a conversation. A protein is never flagged on a population reference range alone; the comparison is always to the patient’s own baseline.

Insulin demand fell, adipokines moved toward a leaner profile, and appetite hormones shifted in the direction expected on a GLP-1. This is the clearest response in the report and is concordant with the clinic HbA1c and fasting glucose.

This is the panel to discuss. IGF-1 fell, follistatin-like 3 rose and two bone-turnover markers rose, a pattern consistent with reduced lean mass or reduced mechanical loading during rapid weight loss. The clinic grip-strength value also dropped. None of this is diagnostic; it is a reason to measure body composition before the next dose step.

What to discuss, what to measure next, what this report does not do.

Suggested discussion points

  • Lean mass. IGF-1 down 17%, FSTL3 up 27%, sclerostin and TRAP5b up, grip strength 26 → 24 kg, 7.7 kg lost in 12 weeks. Consider DXA (or BIA if DXA is unavailable) before stepping to 1.7 mg; protein intake ≥ 1.2 g/kg and resistance training 2–3×/week.
  • Gut epithelium. I-FABP rose during the nausea weeks and had not returned to baseline by Week 12. Re-check once the dose has been stable for four weeks. If GI symptoms return at the next step, this is the marker to watch.
  • Metabolic response is strong. Fasting insulin, C-peptide, leptin, FGF21 and FABP4 all fell; adiponectin and IGFBP-1 rose. HbA1c is now below the prediabetes threshold.
  • GDF15 drifted up but stayed inside the noise band. Common on metabolic drugs; no action, re-measure.

Next measurement window

Week 16–18 · one home sample, after ≥ 4 weeks at a stable dose.

The next sample will be run with this patient's reserved baseline aliquot on the same plate, so the comparison stays within one calibrator lot.

Questions the patient may ask

  • "Is my liver better?" — ALT and liver-fat proteins fell; fibrosis markers were unchanged, which is expected at 12 weeks.
  • "Am I losing muscle?" — The pattern is consistent with it; a scan answers it.
  • "Is my heart OK?" — Endothelial markers improved; troponin and NT-proBNP from the clinic lab are normal.

What this report does not do

It does not diagnose. Proteomic values are Research Use Only. Any result that would change management should be confirmed with an accredited clinical test.

It does not report a biological age. No published proteomic clock is valid on this platform without re-calibration, so none is shown. Organ response indices are within-patient, sign-and-magnitude readouts.

It does not compare patients. Every flag is relative to this patient's own baseline and noise. Population reference intervals are shown only for the accredited clinical values.

Companion. The patient can ask the companion about any line in this report in plain language. The companion explains and prepares questions; it does not change doses. Anything requiring a medical decision is routed back to the ordering clinician.

Capillary whole blood, volumetric tips (20 µL), air-dried and stored at room temperature; four samples returned together at Week 12. Eluted per the panel's protocol (150 µL buffer per tip, 1 h, protease inhibitor).
Batching
All four timepoints for this patient on one plate, same bead lot, same calibrator lot, same run. One aliquot per timepoint held in reserve at −80 °C.
Noise band
Per-protein within-person SD estimated from on-plate technical replicates and cohort baseline variance ("borrowed noise"). A change is reported when it exceeds 1.5× that SD; the direction is then compared to the program's intended direction.
Detectability
781 assays were above LLOQ in all four of this patient's samples and are eligible for reporting; 92 were quantifiable in some timepoints only; 190 were below LLOQ throughout (predominantly intracellular proteins, expected in dried capillary blood).
Sample QC
All four samples passed: haemolysis index low (4 / 4), dwell time 1–84 days recorded, reference sample co-shipped and within tolerance.
Regulatory
Research Use Only. Not CLIA-returnable. Clinical anchors from an accredited laboratory.

Largest whole-panel movers outside the curated panels

Of the proteins not shown on the organ pages, these moved most relative to the patient's own noise. Exploratory; included so nothing large is hidden by panel curation. Detect = timepoints above LLOQ.

Within-person CV, LLOQ and detectability come from the per-target validation table of the platform. Gene symbols are the assay identifiers of the panel.
